What we collect
When you use our booking or enquiry forms we collect the details you provide – such as your name, contact details, date of birth (optional), the service you're interested in and your message.
Why we use it
We use this information only to respond to your request, arrange your appointment and provide pharmacy services. Our lawful basis is your request to us and, for health information, the provision of health care (UK GDPR Articles 6(1)(b) and 9(2)(h)).
How it's sent and stored
Form submissions are delivered to the pharmacy by email via our form provider (Web3Forms). We keep records in line with NHS records-management guidance and delete enquiries we no longer need. We never sell your data or use it for marketing without your consent.
Your rights
You can ask to see, correct or delete your information by contacting us on 020 7622 9020 or in person. You can also complain to the Information Commissioner's Office (ico.org.uk).
